Win a smile makeover worth up to $10,000!

Rules......

1. There is only one (1) Grand Prize. The prize consists of a smile makeover worth up to $10,000. Actual value amount depends on the amount of dental work needed to achieve a beautiful, healthy smile up to and not over $10,000 (ten thousand dollars).

2. One (1) entry per person

3. No purchase necessary

4. Must be 18 years old or older to enter and win.

5. Prize is non-transferable

6. Winner agrees to allow us to use their name and image in future advertising campaigns and agrees to sign a release to that effect.

7. Winner will be responsible for all taxes

8. Drawing will be held on August 31, 2009

9. You do not need to be present to win

10. Winners will be notified by phone, mail or e-mail by September 1, 2009
